
Essential Supplies Program Guidelines
Connecting Care Niagara’s Essential Supplies Program ensures that seniors and those with disability across the Niagara Region have access to basic health and hygiene products in times of need. Items like incontinence briefs, toothpaste, shampoo, lotion, toilet paper, and laundry soap can be difficult to access—especially during urgent situations when family, finances, or community support aren't available. This program is designed to step in during those critical moments, offering timely and respectful support through care packages and donated items.
How it works

Application – A simple online form can be submitted by individuals or healthcare providers.

Review – Our volunteer committee reviews each application within 3–5 business days.

Approval – Based on available inventory, eligibility criteria, and urgency, we determine what can be provided.

Delivery – Approved applicants receive their item(s) directly to their residence or designated contact or pickup can be arranged.
Eligibility Criteria
Applications are assessed based on:

Residence in Niagara Region (home or care facility).

Urgent need due to hardship, lack of support, or difficulty accessing programs.

Requested items falling within our supported product categories.
Our Approval Process
Each request is rated on five key factors:

Urgency of Need

Vulnerability of the Recipient

Availability of Other Resources

Clarity and Completeness of the Application

Alignment with Program Goals
Applications are scored and prioritized accordingly:

High Priority (16–20) – Approved and fulfilled immediately.

Medium Priority (11–15) – Fulfilled if supplies are available.

Low Priority (6–10) – May be waitlisted or require follow-up.

Not Eligible (0–5) – Compassionate decline with alternative referrals.
