It is not uncommon for individuals to feel lost on their healthcare journey. Our responsibility is to educate the community about additional resources available, empowering them to enhance the quality of life for themselves and their loved ones.

Urinary Tract Infections

Our UTI Initiative provides free UTI test kits to help individuals detect infections early, reducing hospital visits and improving health outcomes.

Essential Supplies Program

Our Essential Supplies Initiative ensures seniors in need receive essential personal care products like briefs, toothpaste, and moisturizers to support their well-being.

Community Guide Book

This community guide book was created to make navigating care in the Niagara Region easier for everyone.Whether you’re a healthcare provider, caregiver, or community member, this guide brings together services, programs, and supports in one accessible place. By offering this resource, Connecting Care Niagara aims to strengthen collaboration, reduce barriers, and ensure people are connected to the right care at the right time.
